Support Coordination
We at GreenLight Disability Support Services know how important it is that you are connected with the right supports and services. This is imperative to realizing your individual goals and aspirations. Keeping this as our topmost priority, our excellent Support Coordinators are dedicated to guiding you as you familiarise yourself with your NDIS plan and its implementation.
Our Support Coordinators will plan and manage your NDIS funding with you and help you to put services and supports in place to best meet your needs. They will be your point of contact when you choose to alter, adjust or increase your support. They can also work in coordination with your family and/or your carers to ensure that your goals are achieved.
The following are some of the services that our Support Coordination Team can provide:
- Manage your NDIS plan
- Assist in arranging setup of service and service bookings
- Ensure consistent delivery of service
- Increase your well-being capacity
- Assist you to achieve day-to-day, short-term and long-term goals
- Increase your independence
Specialist Support Coordination
GreenLight Disability Support Services provides Specialist Support coordination. This service is for participants who may require a higher level of support coordination. This service is designed to manage any and all challenges that you face in your daily life. We will support you by connecting you with the services and providers that support your individual goals and needs.
Our Specialist Support Coordination team supports you while you address the challenges in your environment, build your capacity and resilience and, also assist you by connecting you to appropriate supports.
The following is a list of all the higher levels of support that encompasses our Specialist Support Services:
- psychological and mental health support
- accommodation arrangements
- ongoing support from a trained and knowledgeable Support Coordinator
- accessing health, education or justice services
Psychosocial Recovery Coaching and Mental Health Support Worker
If you have been diagnosed with a psychosocial disability or mental health illness and if it significantly impacts your day to day living and your ability to participate in community life, then the NDIS can now offer you support to aid in your recovery. You are able to choose the supports you require to achieve your personal recovery goals.
GreenLight Disability Support Services has experienced and dedicated Recovery Coaches. They will assist you by helping you to take more control of your life. They will also support you to tackle and manage complex day-to-day challenges. We work with you and support you to build capacity and resilience through strong relationships. We want you to gain social and economic independence and confidence for an increased quality of life. We help you to navigate through the NDIS service system.
The following are a list of services provided to you in the Recovery Plan:
- Identify strengths and barriers
- Enable you to identify areas of life where you are given a choice and areas where the choice lies with someone else
- Map Identified goals with available resources and services
- Identify key contacts
In-home support
Our In-Home Support services are tailored to suit your individual needs and goals. Our services are tailored to suit every age and ability, and we connect you with all the services you will need to live in your own home and community with complete independence. This includes everything from helping people understand the disability care system to connecting you with services so you can live in your own home and engage with your community.
We can also assist you with household tasks if your disability prevents you from performing them. We support you to undertake domestic activities while helping you build your skills so that you can lead your life with more independence.
The following are some of the household services we provide:
- vacuuming and mopping floors
- changing linen
- laundry and ironing
- dusting and wiping surfaces
- kitchen cleaning including sinks, taps and microwaves
- cleaning toilets, showers and baths
- lawn mowing and yard maintenance
- path and driveway cleaning
- personal hygiene (e.g., showering, dressing and grooming)
- grocery shopping and meal preparation
- budgeting
- paying bills
- recreational and social outings
- assistance with mobility and exercise
- assistance with transportation to health appointments
Life-Skills Development
Developing Life Skills allows people with disabilities to tackle life’s great challenges with ease. And we at GreenLight Disability Support Services believe that we can help you develop your life skills which will help you increase your independence and confidence. Our services are available individually or in a group setting, the choice is yours.
We provide a range of different support services and activities to build new life skills or develop pre-existing skills. These include:
- Budgeting skills
- Cooking
- Cleaning
- Public transport training
- Attending appointments
- Shopping
- Paying bills
- Nutrition
- Numeracy
We seek to make you as independent as possible. We offer a range of training and development activities that are aimed at building your capacity. This assists in managing your day‐to‐day life, through the development of your knowledge, skills and networks of support. We try to create groups where age or areas of interest match and then run activities throughout the week and during weekends. Over time, we seek to achieve the following goals that will make a positive impact on your life:
- Improved daily living
- Increased social and community participation
Community Participation
Community Participation is one of the important Core Supports available to people with a disability. It is a service that is designed to help support you to meet your goals in relation to improving community interaction and participation. The Community Participation program at GreenLight Disability Support Services will help you to learn new skills or build on pre-existing skills to ensure active participation in the community. We support you to participate in community, social or recreational activities, and in employment.
Under our Community Participation Program, you have the following opportunities:
- participating in social groups
- attending personal development, vocational or educational courses
- visiting your local library
- going on holidays, camps or outings
- attending movies or concerts
- attending community events
- going to health or medical appointments
We also provide assistance with transport to and from an activity. We provide assistance with communication, personal care etc while you are participating in or attending events.
Group Activites
Group Activities are more fun with Greenlight Disability Support Services. We can host a range of group/centre activities and programs that are a great way to build friendships and acquire new skills. Our main aim is to increase your capacity and increase your social and community participation opportunities. We encourage you to engage in activities independently or as a part of a group in our workshops.
Our Group Activities includes the following:
- Music; listening to, playing and attending music events
- Art and craft activities
- Tv and movies
- Social meals
- Games; group games/computer games
- Community gardening
- Solo and team sport, assistance with exercise
Transport Assistance
For people with a disability, travelling on your own can be a very daunting task. But with the services provided to you by GreenLight Disability Support Services, travel is no longer an issue. We assist you in gaining greater confidence and independence by providing effective solutions to your travel problems.
We provide effective public transport training and also support staff who ensure that you are safe and comfortable at all times. We increase your independence through public transport training by helping you to learn public transport routes, bus/train timetables, and buying, recharging and using your MetroCard.
Our goal is to help you get around and engage in your community independently. With this purpose in mind, we offer the option of private transport too. Whether you have to travel to school or work, to medical appointments, or for shopping, you can book our transport services. The bookings must be scheduled in advance and can be used for:
- Medical appointments
- Work
- Place of education
- Local shopping centre
- Visit to the local chemist
- Community events, groups or clubs
- Park or library.
Therapeutic Support
The main goal of NDIS Therapeutic Support is to help you in improving your independence. This will enable your daily participation in practical activities like mobility and movement, personal care, community living, speech and communication. Our therapeutic supports will help you to improve your daily living skills, support and maintain increased physical mobility and wellbeing.
The following are included in our NDIS Therapeutic Support Program:
- Occupational therapy – will help you to develop your functional skills to improving your day-to-day participation and independence. This will also include assessments for daily activities such as showering and cooking.
- Behaviour support – is designed to help you achieve your goals surrounding your mental health, behaviour and relationships. Assistance is provided so that you, your family and or your carers can implement positive behaviour strategies.
- Speech therapy – Speech pathologists assist you with communication. Speech pathologists treat communication disorders, including difficulties with speaking, listening, understanding language, reading, writing, social skills, stuttering and using voice.
- Physiotherapy – Physiotherapy is treatment which can restore, maintain, and make the most of a patient’s mobility, function, and well-being.
Specialist Disability Accommodation
A small percentage of NDIS participants may be eligible to receive funding for Specialist Disability Accommodation and assistance with daily living costs. The main aim of this funding is to provide people with disabilities, greater independence and choice to decide how they would like to live their lives. This will provide the opportunity to move out of their family home or from their group home and live independently in mainstream housing; in an area of their choosing.
We at GreenLight Disability Support Services provide you with a range of housing options so that you can live with greater independence in a caring and supportive environment. Our Support Coordinators will assist and support you; as you explore all your options and choose the services and supports that will help you achieve your individual goals.
The following is a brief outline of the housing options available to you:
- Supported Independent Living [SIL]: This support is available in shared living arrangements and includes services, like assistance with personal care, day to day living supports or meal preparation.
- Independent Living Options [ILO]: This option gives you the choice to choose which service suits your individual needs. This is a two-stage service that gives you the option of exploring and designing supports that best suit your requirements.
- Medium Term Accommodation [MTA]: is an intermediary support service that is available to those individuals that are awaiting a confirmed long-term accommodation vacancy.
- Short Term Accommodation [STA]: is available to those individuals that require a short stay away from their usual accommodation. You have the option of staying by yourself or in a group setting.
- Respite Care: This time-limited service is available to you or your family when you need a break from your usual routine or in case of an emergency.
